A Civil Cover Sheet must be submitted along with any new civil complaints submitted to the Court.

• Complete all sections of the document to the best of your abilities.
• Select only one Nature of Suit code (Section IV).
• Sign the document with a “wet” (handwritten) signature and date. (Typed and/or electronically signed documents are not permitted.)
